PWV可以反映动脉弹性的情况,PWV越快,动脉的弹性越差,僵硬度越高。baPWV 反映大动脉和中动脉系统的弹性状态。一项横断面研究采用Fram ingham公式计算表明主动脉PWV与心血管危险相关[7],PWV是评价主动脉硬度的经典指标,PWV改变是动脉结构与功能异常的总体反映。已有研究表明,主动脉僵硬度升高与冠心病的发生密切相关[8],Kim Sutton等[9]对2488例社区健康老年患者的流行病学调查显示: PWV作为动脉僵硬度的衡量指标,与心血管事件,冠心病,卒中发病率密切相关。因此,PWV通过反映大动脉弹性,可能是预测冠心病发病及冠状动脉病变程度的较好指标。Raison等[10]研究则表明,对23例高血压合并高胆固醇血症患者给予阿托伐他汀12周后主动脉段PWV显著降低8%。本研究表明,血脂康也可以明显改善PWV。与此同时,发现PWV与FMD呈直线负相关,应用超声检测FMD是反映局部动脉弹性,肱动脉的顺应性越好,FMD越大,肱动脉顺应性越差,FMD越小。 PWV反映全身动脉弹性,PWV与大动脉僵硬度直接相关,动脉管壁的顺应性越大,PWV越小,动脉管壁的顺应性越小,PWV越大。该研究发现此两项指标反映的动脉弹性情况是统一的。刘东红等人在研究冠心病患者时,也发现baPWV与FMD呈直线负相关[11]。
综上所述,心肌梗死患者长期应用血脂康可以改善外周动脉内皮舒张功能,改善动脉弹性,从而推断出血脂康可以改善冠状动脉粥样硬化,为冠心病的预防和治疗指导用药。
