【摘要】 目的 探讨代谢综合征(MS)患者血管内皮功能与左心室重构情况及两者的相关性。方法 选择43名健康人为对照组,92例MS患者分为3组:肥胖及血脂紊乱合并原发性高血压而血糖正常组(MS1组);合并2型糖尿病血压正常组(MS2组);合并原发性高血压和2型糖尿病组(MS3)。使用彩色超声多普勒分别测量血管内皮功能及左心室结构相关参数,分析两者的相关性。结果 MS1、MS2、MS3组内皮依赖性血管舒张功能(EDD)较对照组明显降低 (P<0.01);MS3组EDD障碍较MS1、MS2组更明显(P<0.05)。MS1、MS2、MS3组舒张末期室间隔厚度、舒张末期左心室后壁厚度、左心室质量、左心室质量指数(LVMI)均明显高于对照组(P<0.05或P<0.01);并且MS2、MS1、MS3的各参数值分别依次增加,3组间差异有统计学意义(P<0.05或P<0.01)。EDD与LVMI呈负相关(偏相关系数为-0.306,P<0.05)。结论 MS的危险因素与血管内皮功能紊乱和左心室重构相关。血管内皮功能紊乱与左心室重构有关。

【Abstract】 Objective To investigate the endothelial dysfunction and its relationship with left ventricular remodeling in patients with metabolic syndrome (MS). Methods Ninety瞭wo patients with MS and 43 control subjects were recruited for this study. The patients with MS were divided into the following 3 groups, essential hypertension with normal blood glucose (MS1), type 2 diabetes mellitus with normal blood pressure (MS2) and essential hypertension with type 2 diabetes mellitus, obesity and dyslipidemia (MS3).Parameters related to endothelial function and left ventricular structure were measured by echograph. Results Endothelium瞕ependent dilatation (EDD) in MS1, MS2, MS3 was decreased significantly compared with control subjects (all P<0.01).EDD in MS3 was lower than in MS1 and MS2 (P<0.05). Left ventricular posterior wall thickness, interventricular septal thickness,left ventricular mass and left ventricular mass index (LVMI) in MS2, MS1 and MS3 was increased significantly compared with control subjects (P<0.05 or P<0.01), and the parameters mentioned above were increased in the order of MS2, MS1 and MS3 and significant differences were found among MS2, MS1 and MS3 (P<0.05 or P<0.01). EDD was negatively related to LVMI (partial correlation coefficient was -0.306, P<0.05). Conclusions MS is related with endothelial dysfunction and left ventricular remodeling, and endothelial dysfunction is related with left ventricular remodeling. 中国大学排名

代谢综合征(metabolic syndrome,MS)为糖脂代谢紊乱、原发性高血压和肥胖的聚集体。研究表明,聚集的多重危险因素协同作用,对血管内皮细胞形态及功能的复合影响更大,导致的内皮功能紊乱(endothelial dysfunction,ED)更严重,对靶器官造成的危害也更大,而ED反过来又促进了上述病理过程的发生和发展[12]。因此,有必要将MS血管内皮功能及靶器官损害联合进行研究。
1 对象和方法
1.1 研究对象
选择我院2007年7月至2008年7月住院MS患者92例,分为合并原发性高血压而血糖正常组(MS1)29例(男15例,女14例),平均年龄(50.1±5.6)岁(40~65岁);合并2型糖尿病而血压正常组(MS2)31例(男14例,女17例),平均年龄(51.7±6.3)岁(42~71岁);合并原发性高血压、2型糖尿病、肥胖及血脂紊乱组(MS3)32例(男17例,女15例),平均年龄(52.4±7.3)岁(44~78岁)。健康对照组(NC)43名,均选自同期于我院体检健康者(男25例,女18例),平均年龄(50.6±4.4)岁(37~72岁)。
1.2 纳入标准
MS按2004年中华医学会糖尿病学分会提出的诊断标准,即符合下列中3项或更多指标即可诊断:(1)体重指数(body mass index,BMI)>25 kg/m2。(2)空腹TG≥1.7 mmol/L,和/或HDL睠降低:男性<0.9 mmol/L,女性<1.0 mmol/L。(3)血压增高≥140/90 mmHg,和/或已确诊为原发性高血压并经治疗者。(4)空腹血糖≥6.1 mmol/L,和/或餐后2 h血糖≥7.8 mmol/L,和/或已确诊为糖尿病并经治疗者。原发性高血压按1999年世界卫生组织高血压联盟(WHO/ISH)指南的标准,除外继发性高血压。2型糖尿病按WHO(1999年)糖尿病诊断标准,除外1型糖尿病和继发性糖尿病。
