螺内酯对老年舒张性心力衰竭患者血清Ⅰ型前胶(2)

步研究发现老年舒张性心功能不全患者每日口服螺内酯25 mg/d 4个月后血清PICP水平明显下降，超声心动图E/A比值明显上升，患者舒张功能明显改善[6]。本研究亦显示舒张性心功能不全老年患者口服螺内酯20 mg/d 6个月后血清PICP水平明显下降，超声心动图E/A比值明显上升，左心房内径缩小、室间隔及左心室后壁厚度降低，与国外报道一致。本研究结果还显示患者左心室舒张末期内径在治疗前后无明显变化，考虑可能与治疗前患者左心室无明显扩大、患者数量少、观察时间短有关，具体机制有待进一步探讨。在治疗期间监测血钾均在正常范围，未发现血钾增高患者。
    ADL受体拮抗剂螺内酯与ACEI制剂联用可以进一步降低血清PICP浓度，减少Ⅰ型胶原的合成，减轻心肌纤维化，从而改善心脏的舒张功能，是一种改善心脏舒张功能安全有效的药物。参考文献：
